Conservative Aesthetic Bridge and Crown Replacement with Soft Tissue Contouring

Lisa presented with dissatisfaction over the aesthetic appearance of her anterior bridge. She had been wearing a long-standing gold bridge (UL4–UL2), replacing UL3, with visible aesthetic concerns including mismatched shades, exposed margins, and poor gingival contour. The patient specifically expressed concern over the visibility of the metal components and disharmony within the smile line.

Despite the poor appearance, the existing prosthesis had lasted over 25 years without mechanical failure—a testament to its structural design. Lisa did not want an implant, and given the prosthesis’ longevity, a conservative restorative approach was selected.

Outcome

This case demonstrates how thoughtful execution of a minimally invasive treatment plan can achieve a high-impact result. Without altering the original bridge design, we replaced the crown and pontic unit (UL3–UL4) using a carefully colour-matched ceramic restoration and subtle soft tissue contouring to enhance the gingival profile around the UL3 pontic.

 

Through minor tissue shaping and strategic shade selection—guided by calibrated photography techniques—the final result blended seamlessly into Lisa’s natural dentition. The outcome restored her confidence without surgical intervention, maintaining structural integrity while vastly improving aesthetics.

How we did it

Diagnosis and Treatment Plan

  • Primary concern: Poor aesthetics of an old metal-ceramic bridge, visible margins, and unnatural shade
  • Diagnosis:
    • Aesthetic failure of existing UL4–UL2 bridge
    • Poor soft tissue adaptation around UL3 pontic
  • Plan:
    • Maintain existing bridge configuration (no implant placement)
    • Remove and replace the UL3–UL4 bridge with a new ceramic restoration
    • Perform soft tissue sculpting around the pontic site to support ideal emergence profile

This case exemplifies the value of preserving existing prosthetic design when appropriate, especially when the structural integrity of the prosthesis has already demonstrated long-term success. Rather than opting for a more invasive approach such as implant placement, we achieved a highly aesthetic and functional outcome through subtle refinements in soft tissue and shade matching.

Careful pontic site development, gingival shaping, and prosthetic craftsmanship allowed us to deliver a restoration that was indistinguishable from the surrounding dentition. This case also highlights how photography and digital planning—often overlooked in smaller cases—can dramatically improve the final result.
